 OBJETIVO: Comparar la precisión de la ecografía y la RM artrografía de imágenes (ARM) en la detección de los desgarros del manguito de los rotadores con hallazgo artroscópico utilizado como patrón de referencia.
MÉTODOS: La ecografía y hallazgos ARM de 51 hombros que se sometieron a la cirugía artroscópica se analizaron de forma prospectiva. Dos médicos ortopédicos de forma independiente realizaron la ecografía y la interpretaron los hallazgos en la oficina. El tamaño de la lágrima medido en la ecografía y MRA se comparó con el tamaño medido a cirugía utilizando coeficientes de correlación de Pearson (r). La sensibilidad, especificidad, exactitud, valor predictivo positivo, valor predictivo negativo y la tasa de falsos positivos se calcularon para un diagnóstico de desgarros parciales del manguito rotador y de espesor total. El coeficiente kappa se calculó para verificar el acuerdo entre observadores.
RESULTADOS: La sensibilidad de la ecografía y la MRA para detectar desgarros de espesor parcial fue de 45.5 y 72.7%, y el de las lágrimas de espesor total fue 80,0 y 100%, respectivamente. La precisión de ultrasonograpy y MRA para detectar desgarros de espesor parcial fue del 45,1 y el 88,2%, y el de las lágrimas de espesor total fue del 82,4 y 98%, respectivamente. El tamaño de desgarro se mide basándose en el examen ecografía mostró una pobre correlación con el tamaño medido en la cirugía artroscópica (r = 0,21, p <0,05). Sin embargo, el tamaño de lágrima estimado por MRA mostró una fuerte correlación (r = 0,75, p <0,05). El coeficiente kappa fue de 0,47 entre los dos examinadores independientes.
CONCLUSIONES: La precisión de la ecografía en el consultorio para los cirujanos ortopédicos para principiantes para detectar de espesor completo manguito rotador fue comparable a la de MRA, pero fue menos preciso para detectar desgarros de espesor parcial y la medida del tamaño desgarrado. Abstract

PURPOSE: To compare the accuracy of ultrasonography and MR arthrography (MRA) imaging in detecting of rotator cuff tears with arthroscopic finding used as the reference standard.
METHODS: The ultrasonography and MRA findings of 51 shoulders that underwent the arthroscopic surgery were prospectively analysed. Two orthopaedic doctors independently performed ultrasonography and interpreted the findings at the office. The tear size measured at ultrasonography and MRA was compared with the size measured at surgery using Pearson correlation coefficients (r). The sensitivity, specificity, accuracy, positive predictive value, negative predictive value and false-positive rate were calculated for a diagnosis of partial-and full-thickness rotator cuff tears. The kappa coefficient was calculated to verify the inter-observer agreement.
RESULTS: The sensitivity of ultrasonography and MRA for detecting partial-thickness tears was 45.5 and 72.7 %, and that for full-thickness tears was 80.0 and 100 %, respectively. The accuracy of ultrasonograpy and MRA for detecting partial-thickness tears was 45.1 and 88.2 %, and that for full-thickness tears was 82.4 and 98 %, respectively. Tear size measured based on ultrasonography examination showed a poor correlation with the size measured at arthroscopic surgery (r = 0.21; p < 0.05). However, tear size estimated by MRA showed a strong correlation (r = 0.75; p < 0.05). The kappa coefficient was 0.47 between the two independent examiners.
CONCLUSIONS: The accuracy of office-based ultrasonography for beginner orthopaedic surgeons to detect full-thickness rotator cuff tears was comparable to that of MRA but was less accurate for detecting partial-thickness tears and torn size measurement. Inter-observer agreement on the interpretation was fair. These results highlight the importance of the correct technique and experience in operation of ultrasonography in shoulder joint.

ANTECEDENTES: La resonancia magnética (MRI) es de alta precisión en el diagnóstico de espesor completo desgarros del manguito rotador, pero tiene las limitaciones de alto costo y los inconvenientes basados ​​en el paciente. Acuerdo entre la ecografía y la resonancia magnética ha mejorado con el aumento de número de ecografías realizadas. La tasa máxima de aumento de la sensibilidad se produjo por el ultrasonido 50a para ambos cirujanos. La tasa máxima de aumento de la especificidad se produjo por el ultrasonido 75a para ambos cirujanos. Precisión general para detectar las lágrimas supraespinoso de espesor total se estabilizó después de 100 ecografías.
CONCLUSIONES: La mayoría de aprender a detectar las lágrimas supraespinoso espesor completo por ultrasonido se produjeron en los primeros 50 casos, los cirujanos. La capacidad de distinguir con precisión un tendón supraespinoso normal mediante ecografía llevó más tiempo, que ocurre dentro de los primeros 75 casos. Abstract

BACKGROUND: Magnetic resonance imaging (MRI) is highly accurate in diagnosing full-thickness rotator cuff tears but has the limitations of high cost and patient-based inconveniences. Diagnostic ultrasound of the rotator cuff has become popular, but its accuracy is operator dependent. We hypothesized that the incremental learning associated with initiation of shoulder ultrasonography in the orthopedic office setting could be quantified.
MATERIALS AND METHODS: Ultrasound was performed by 2 surgeons in different patient populations each consisting of 200 patients within the clinical setting. Analysis of the operator learning curves based on sensitivity, specificity, positive predictive value, negative predictive value, and κ agreement scores was performed with MRI used as the reference standard.
RESULTS: Surgeon 1 detected 67 (ultrasound) of 72 (MRI) full-thickness supraspinatus tears, and surgeon 2 detected 92 (ultrasound) of 95 (MRI) full-thickness tears. Agreement between ultrasound and MRI improved with increasing number of ultrasounds performed. The maximal rate of increase in sensitivity occurred by the 50th ultrasound for both surgeons. The maximal rate of increase in specificity occurred by the 75th ultrasound for both surgeons. Overall accuracy to detect full-thickness supraspinatus tears plateaued after 100 ultrasounds.
CONCLUSIONS: The majority of learning to detect full-thickness supraspinatus tears by ultrasound occurred in the surgeons' first 50 cases. The ability to accurately distinguish a normal supraspinatus tendon by ultrasound took longer, occurring within the first 75 cases. The majority of learning before accuracy plateaued occurred within the first 100 ultrasounds. The recommended operator experience necessary to evaluate the supraspinatus tendon before clinical application was 100 ultrasounds.

 OBJETIVO: El objetivo de este estudio fue analizar los resultados de sutura artroscópica reparación de puentes de grandes desgarros del manguito de los rotadores.
Métodos: Treinta y seis pacientes con desgarros masivos del manguito rotador que habían sido reparados por artroscópica sutura reparación de puentes se inscribieron en este estudio. La media del período de seguimiento después de la cirugía fue de 37,6 ± 8,9 meses (rango, 25-56). Artroscópica en masa reparación de puentes de sutura se utilizó en lágrimas deslaminadas. La integridad del manguito rotador reparado fue examinado por ecografía 4,5 meses, 1 año y 2 años después de la cirugía. Los resultados funcionales fueron evaluados utilizando la American Hombro y Codo Cirujanos (ASES) Partitura de hombro, la puntuación de Constant, el rango de movimiento y la fuerza muscular.

Abstract

PURPOSE: The purpose of this study was to analyze the outcomes of arthroscopic suture bridge repairs of massive rotator cuff tears.
METHODS: Thirty-six patients with massive rotator cuff tears that had been repaired by arthroscopic suture bridge repair were enrolled in this study. The mean follow-up period after surgery was 37.6 ± 8.9 months (range, 25 to 56). Arthroscopic en masse suture bridge repair was used in delaminated tears. The integrity of the repaired rotator cuff was examined by ultrasonography 4.5 months, 1 year, and 2 years after surgery. Functional outcomes were evaluated using the American Shoulder and Elbow Surgeons (ASES) shoulder score, Constant score, range of motion, and muscle power.
RESULTS: Twenty-seven cases (75%) showed complete healing on serial ultrasonography, whereas recurrent tears were detected in the remaining 9 cases (25%). Five retears were smaller than, and the other 4 were the same as or larger than, the initial tear. Overall mean functional scores and values improved after surgery. Although the 4 larger retears had a poorer outcome with respect to ASES score than the healed group (P = .005), the Constant score for these 4 retears did not significantly differ at the last follow-up (P = .175). Mean ASES and Constant scores for the larger tear group were also relatively satisfactory (64.2 and 63.0, respectively). Fatty degeneration of the supraspinatus on preoperative magnetic resonance images was associated with a high retear rate (P = .003, odds ratio = 13.664).
CONCLUSIONS: Massive rotator cuff tears can be treated successfully by arthroscopic en masse suture bridge repair with satisfactory results. Larger retears had some adverse effects on functional outcome, whereas patients with smaller retears did not significantly differ in functional outcome from the healed group. Fatty degeneration of the supraspinatus on preoperative magnetic resonance images is a poor prognostic indicator.

La radiografía, la resonancia magnética (MRI) y la ecografía se utilizan habitualmente para el diagnóstico de la osteocondrosis hombro y osteocondritis disecante (OC / OCD) en perros, sin embargo, hay una falta de información publicada sobre la sensibilidad de diagnóstico relativas de estas modalidades. Abstract

Radiography, magnetic resonance imaging (MRI), and ultrasonography are commonly used for diagnosis of shoulder osteochondrosis and osteochondritis dissecans (OC/OCD) in dogs, however there is a lack of published information on the relative diagnostic sensitivities of these modalities. The purpose of this prospective study was to compare diagnostic sensitivities of these modalities for detecting shoulder OC/OCD in a group of dogs, using arthroscopy as the reference standard. Inclusion criteria were history and clinical findings consistent with osteochondrosis and/or osteochondritis dissecans involving at least one shoulder. With informed client consent, both shoulders for all included dogs were examined using standardized radiography, ultrasonography, MRI, and arthroscopy protocols. One of three veterinary surgeons recorded clinical and arthroscopic findings without knowledge of diagnostic imaging findings. One of two veterinary radiologists recorded diagnostic imaging findings without knowledge of clinical and arthroscopic findings. Eighteen client-owned dogs (n = 36 shoulders) met inclusion criteria. Diagnostic sensitivity, specificity, and accuracy (correct classification rate) values for detecting presence or absence of shoulder osteochondrosis/osteochondritis dissecans were as follows: radiography (88.5%, 90%, 88.9%), ultrasonography (92%, 60%, 82.6%), and MRI (96%, 88.9%, 94.4%). Odds of a correct diagnosis for MRI were 3.2 times more than ultrasonography and two times more than radiography. For MRI detection of lesions, the sagittal T2 or PD-FAT SAT sequences were considered to be most helpful. For radiographic detection of lesions, the additional supinated-mediolateral and pronated-mediolateral projections were considered to be most helpful. Findings from the current study support more evidence-based diagnostic imaging recommendations for dogs with clinically suspected shoulder osteochondrosis or osteochondritis dissecans.
